Consent to the processing of personal data

In accordance with Article 7 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter the “GDPR”).

By giving this consent, I agree to the processing of my personal data by Stabilplastik spol. s r.o., company ID (IČO): 25619578, VAT ID (DIČ): CZ25619578, with its registered office at 5. května 457, 250 64 Měšice, registered in the Commercial Register kept by the Municipal Court in Prague, Section C, File 55360 (hereinafter the “Company”) in the following scope:

First name, surname, e-mail, address, telephone number, business name

This consent is valid only if my personal data are processed solely to the extent necessary to achieve the purpose of processing stated below and in accordance with applicable legislation.

The consent is given for the purpose of approaching me with product offers.

I agree to the processing of my personal data by the Company for the duration of our cooperation and for 1 year after its end.

The Company is entitled to use my personal data only in accordance with the above purpose, or for the legitimate needs of state supervisory authorities and law enforcement authorities.

The Company is further entitled to disclose my personal data only to entities cooperating with the Company to achieve the primary purpose for which this consent is given. The Company undertakes to conclude with such entities an agreement containing the same conditions for the processing of my personal data.

Processing will be carried out in accordance with the relevant legal regulations on personal data protection and with the GDPR.

I have been informed that providing the data is voluntary. I have also been informed, in accordance with the applicable legislation:

• of my right to withdraw this consent at any time, without giving a reason,
• of my right of access to these data and the right to have them rectified,
• of my right to erasure of these data if they are processed in breach of the protection defined by the applicable legislation or in breach of this consent, or if the consent has been withdrawn.

I have also been informed that I may exercise these rights by delivering a request to the Company’s address.

I declare that I give this consent freely, that I understand its content, and that my consent has not been made conditional on any other circumstances that would constitute my relationship with the controller (i.e. that my consent is separable from any such relationships) to whom I give this consent to processing.
